The staff at Rocky Mountain Oral X-Ray prides itself in making your office visit as pleasant as possible. In order to better serve you, please read the following:
- Please call our office to schedule an appointment. See maps for business hours & contact information.
- A written or oral prescription is required in order to take the x-rays. Bring the completed form to your appointment.
- Payment is required at the time of the appointment. You will be quoted a fee when you schedule.
- You will be provided with a receipt to submit to your insurance company for reimbursement.
- Your records will be sent by UPS directly to your doctor. The records will remain with your doctor.
- You will need to remove jewelry and piercings around the head area for most procedures.
- Notify technician if you are pregnant or have an infectious illness.
- Please notify us in advance for any disability requiring special assistance. We may need to allow extra time.
- 24-hour notice of cancellation is appreciated and patients more than 15 minutes late may be asked to reschedule.
