COMPLETE THE IMAGING ORDER FORM. Simply download and complete an RMOX Imaging Order Form. Please include any specific comments or special requests. Make a copy for your files and provide a copy to your patient.
SCHEDULE AN APPOINTMENT. Your office or your patient can call with the completed Imaging Order Form to schedule the appointment at one of our convenient locations.
GET AN ESTIMATE OF FEES. Payment is required at the time of the appointment. A fee estimate is quoted when your patient is scheduled. If you prefer, we can directly bill your office, just let us know when you schedule the appointment or note it on the Imaging Order Form.
